Toronto Community Addiction Team (TCAT)

The Toronto Community Addiction Team (TCAT) was developed to improve health and social outcomes for people with complex substance use issues. TCAT provides city-wide mobile intensive case management for people with frequent re-admissions to a withdrawal management system and/or Hospital Emergency Departments. Working from a client-centered approach, we encourage respect, empowerment and dignity within a non-judgemental, anti-oppressive framework.

The process is simple:

  • If you are a frequent user of Withdrawal Management or Emergency Room services, you may receive a referral to TCAT.

  • A TCAT worker will arrange a meeting with you so that you can learn more about the program and ask any questions you may have.

  • If you choose to become a TCAT member, a case manager will work with you to acheive your self-identified goals.

TCAT offers a Peer Training and Development program for TCAT clients. This harm reduction based program focuses on areas of skill building and training, strengthening of support networks, and improved navigation of services.

TCAT is co-lead by St. Michael's Hospital and funded through Toronto Central LHIN.
