  • After Hours Supervisor, Home Care Team - Part-Time
    Reporting to the Senior Manager, Client Care Services; the incumbent is responsible for business related to After Hours service for the provision of all Personal Support and homemaking programs and services. The incumbent will provide intake and coordination of newly assigned and current clients, supervise Personal/Home Support Workers and responds to the urgent inquires made by staff and clients as required when on duty. Hours of work are rotating weekends (Friday 10:00pm to Sunday 10:00pm) and occasional coverage during the week for the Monday to Friday role (5:00pm – 8:30am).

  • Case Manager, Enhanced Follow-Up Support Services
    The Case Manager, Enhanced Follow-up Support Services is part of Urban Health and Homelessness Services (UHHS) and is responsible for providing specialized enhanced follow-up supports to adults and seniors exiting homelessness. Working from a trauma informed, client centered, harm reduction approach, case managers provide one-to-one supports, home visits and telephone contact to assist participants in identifying and achieving self determined goals. Working closely with participants and external service providers, the case manager liaises with and coordinates access to services that assist with housing stabilization and community integration.

  • Community Connect Case Manager
    This program provides continuing care supports for 20 -25 self-identified women who have recently stayed at the MGH WMS. Participants will be offered a range of services and supports based on their individual goals including: short-term case management, accompaniments, peer support, supported referrals, groups and social-recreation opportunities. This program is rooted in a women-centered, trauma informed approach, working from a harm reduction framework, to support self-identified women across the continuum of care with particular focus on critical transitions. Project staff will work in partnership with all WMS staff, as well as external providers, to ease this transition and ensure continuity of care.

  • Community Personal Support Worker
    Provides personal, palliative, housekeeping, respite and other services to the elderly, adults with disabilities and other vulnerable individuals in our community. The Personal Support Worker performs tasks of a confidential nature while providing services to a vulnerable, high-risk client population and is expected to comply with Confidentiality and Conflict of Interest Policies and privacy legislation.

  • Coordinator, Peer Program: Sheltering Sites Overdose Prevention Services - Contract
    The Coordinator will work collaboratively with a team of Harm Reduction Workers and shelter site operators, to establish and support robust peer support initiatives at identified sheltering sites. The Coordinator will assist in the identification, recruitment, training and support of resident peer workers who will work at each site to provide overdose prevention and other harm reduction supports. The Coordinator will ensure peer program participants have the support they need to successfully complete the program and reach their goals as identified in their individual work plans. The Coordinator is responsible for developing and facilitating peer training sessions and peer work activities, jointly with site operators and the harm reduction team.

  • Custodian - Housing - Contract
    The position is responsible for cleaning and maintaining TNG owned sites. Duties include maintenance and repairs of interiors, supply acquisition, garbage removal and upkeep of exterior grounds and walkways as assigned. Assist with maintenance of building equipment and machinery and respond to emergencies as necessary or required. The position will report to the Senior Housing Manager, and is part of CUPE Local 7797. Position will work 35 hours per week; all statutory Holidays and be required to be on-call every Friday-Sunday from 3:30 pm Friday night until 7:30 am Monday morning.

  • Human Resources Generalist
    The HR Generalist provides support to general department functions. This is a dynamic role whereby the incumbent is responsible for administering and supporting all major HR functions, including but not limited to Recruitment and Onboarding, Health & Safety, Labor Relations, Employee Relations, Job Evaluation, data management, HRIS, and HR metrics.   • Mobile Shelter Support Program Shift Lead (4 Positions)
    As an MSSP Shift Lead, you will play a pivotal role in coordinating day-to-day shift activities and operations. Leveraging your own personal experience with homelessness, mental health challenges, and substance use, you will serve as a guiding force, assisting, inspiring, and mentoring both clients and fellow team members on their journeys toward well-being. Utilizing a trauma-informed approach, you will empower individuals to navigate and access services, de-escalate challenging situations, share valuable resources and information, build trust and rapport, facilitate communication with clients, offer social and recreational support, organize and lead client-focused programs as needed, and advocate on behalf of clients when necessary. The MSSP Shift Lead will also accompany clients to appointments. Furthermore, the MSSP Shift Lead will undertake responsibilities such as welcoming and training new team members and collaborating with other teams, including the MSSP Supervisor(s) and the Senior Manager of Mobile Community Health Services. Effective communication between TNG and site operators is crucial to ensure that the needs of our clients are met efficiently. Additionally, you will actively contribute to overdose prevention and response, safety planning, and the enhancement of harm reduction practices through various strategies and advocacy efforts.

  • Personal Support Worker – Supportive Housing - Part-Time Weekend - Contract
    Provide service to clients of the Supportive Housing Department, in client’s homes in a supportive housing, group home or community setting, according to individual care plans.     Job Description: Reporting to the Manager, Children and Youth Services, the Child and Youth Worker will plan and implement an engaging program for the children in the after-school and young women’s programs. This position is responsible for filling in for the regular cook in the event of illness or vacation in a busy drop-in centre. Working as a team member with other staff in the provision of services to the drop-in membership, the relief cook will cook a hot meal for up to 200 members daily, oversee a number of volunteers, and engage in the safe running of a busy kitchen.

  • Senior Manager, Adult Day Program
    The Senior Manager ADP (Senior Manager) is responsible for executing strategies and workplans for all three of The Neighbourhood Group Community Services (TNGCS) Adult Day Programs (ADP) across the City of Toronto. Over 30% of clients speak a primary language other than English, are often frail, and/or marginalized. The incumbent reports to the Director, Home and Community Care and helps them ensure that programs meet relevant industry standards, Funder requirements and TNGCS’s mission. The ADPs are designed to provide caregiver relief and social, recreational, and therapeutic programs for frail elderly and seniors living with mild dementia, the effects of a stroke, and/or other physical and cognitive issues. These same individuals are not able to attend regular community activities and may speak English, Cantonese, Mandarin and/or Portuguese. Participants receive mental and social stimulation in a friendly and safe environment from caring staff. Transportation is provided by TNGCS to elderly clients who live within the catchment area.

  • Trustee Case Manager - Contract
    As a member of the UHHS department, this position provides direct case management supports to adults on a complete range of complex issues, including health, financial, housing, legal and life skills issues. Working from a harm reduction, trauma informed perspective, case managers provide extensive direct one-to-one support, home visit and telephone contact to assist participants in identifying and achieving self-determined goals. Working closely with the client and external service providers involved, the Case Manager liaises with and coordinates access to services including substance use and mental health supports, landlords, health care providers, legal institutions, income supports, housing providers and other community services. As a member of the UHHS department, this position will provide support in the Corner drop-in as well, working closely with the integrated team.

  • Youth Justice Worker
    This position provides services to youth who have come into contact with the youth criminal justice system, providing intake, support, and the implementation of appropriate sanctions and measures, and the fulfilment of legislated reporting requirements. The worker will facilitate court sanctioned or mandated workshops, providing advocacy and/or court supports to youth/parents and supporting wider violence prevention initiatives. They will work within a dynamic team of youth justice workers to decrease violent crime among youth and support those youth at most risk of violence. The position delivers programs internally and within community-based settings that value youth leadership, youth empowerment and self-determined care and development.
