Case Management

Our case managers help participants develop life skills, access education, health care and financial benefits, as well as liaise with housing and other support services. To achieve these goals, participants and case managers work together to develop a service plan and crisis plan, and identify what we need to do to provide the necessary support. We may also refer participants to other services offered by St. Stephen's, or to other organizations.

Our dedicated case managers can help with:

  • Planning/Goal setting
  • Identifying/implementing harm reduction strategies
  • Advocacy
  • Substance use counselling and support
  • Trusteeship through our Voluntary Trustee Program
  • Money management/budgeting
  • Financial entitlements/income assistance (OW/ODSP/CPP)
  • Housing support
  • Social support
  • Assessments and referrals
  • Health (mental, physical, emotional, spiritual)
  • Referrals to primary health care and psychiatry
  • Community accompaniment, such as escorts to meetings, appointments and outings
  • Legal issues
  • Identification
  • Liaising/coordinating with other service providers
  • Creating community connections
